Source text - English Diarrhoeal disease is a major global health issue, particularly in children. There were 7.6 million deaths in children under the age of five in 2010 (57 deaths/1000 live births) 1. Fourteen percent of these or 1.06 million deaths occurred as a result of diarrhoea, of which the majority were focused in the developing world. Most diarrhoeal episodes are self-limiting, and patients who recover often do so without a causative agent identified; in those that do get a diagnosis, rotavirus is recognised as the leading pathogen2. Other viral causes of diarrhoea include norovirus, astrovirus, sapovirus, and enteric adenoviruses; Salmonella, Shigella, Yersinia, Campylobacter, and Vibrio cholerae are commonly regarded as the main bacterial causes.
Vietnam, in South East Asia, has a rapidly growing economy, with urban areas experiencing dramatic increases in socioeconomic standards. Yet, diarrhoea is still estimated to cause substantial morbidity and mortality. In two 1993 publications by Canh et al., cited by Van Man et al. in 2001, total mortality among children less than 5 years old in Vietnam, was estimated to be 42.2 deaths /1000 live births, 15.4% was as a consequence of diarrhoeal disease3. A more recent WHO publication estimated mortality in children younger than 5 years, in Vietnam, to be 23 deaths/1,000 live births, of which 12% was estimated to be due to diarrhoeal disease1. In Ho Chi Minh City, diarrhoea among children aged younger than five years is the second most common cause of hospitalisation. In a recent study conducted in three hospitals in Ho Chi Minh City, rotavirus was recovered in 41.4% of ca. 1,500 hospitalised paediatric diarrhoeal cases. Norovirus was found in 17% of these paediatric diarrhoeal infections, and 2.3% of cases were norovirus-rotavirus co-infections. Bacterial pathogens were responsible for 10.3 % of hospitalised paediatric diarrhoeal disease, while a pathogen was not isolated in approximately 25% (My Phan et al., manuscript in preparation).
Oral rehydration solution (ORS) therapy remains the mainstay of treatment according to the 2005 WHO guidelines for the treatment and management of acute diarrhoea in children. Herein new ORS formulations and zinc supplementation have shown to reduce the duration of diarrhoea and vomiting 4. Furthermore, antimicrobials are also recommended for more severe gastrointestinal infections, and are frequently prescribed to children hospitalised in Ho Chi Minh City with acute infections. In Vietnam, the use of probiotic supplements is also common for the treatment of diarrhoea. The term “probiotic” as it is used today was first introduced by Parker in 1974, as cited by Schrezenmeir and de Vrese in 2001, and is used to define “organisms and substances which contribute to intestinal microbial balance” 5. More specifically, these are typically lactic acid bacteria, supplementary non-pathogenic bacteria and yeasts 6. The effect of probiotics has been investigated in, and summarised for, several diseases and conditions, including acute diarrhoea, lactose maldigestion, antibiotic associated diarrhoea, traveller’s diarrhoea, allergies, Clostridium induced diarrhoea, dental caries, intestinal inflammation in children with cystic fibrosis, upper respiratory tract infections in children, nasal colonisation with pathogens and treatment of inflammatory bowel disease 7. Additional review articles have summarised trials studying the effects of probiotics on the prevention of several other diseases, including the prevention of acute infectious diarrhoea in day-care centres, and the prevention of nosocomial and antibiotic associated diarrhoea 8, 9. In the case of acute diarrhoea, modest reductions in the frequency of diarrhoeal symptoms have been reported when adults and young children, have been treated with probiotics 10 – 12.

Một nghiên cứu gần đây được thực hiện tại 3 bệnh viện ở thành phố Hồ Chí Minh cho thấy rotavirus chiếm 41.4% trong số 1500 ca trẻ nhập bệnh do tiêu chảy. Norovirus được tìm thấy trong 17% các ca nhiễm tiêu chảy ở trẻ em và 2.3% đồng nhiễm norovirus và rotavirus. Vi khuẩn là nguyên nhân gây bệnh cho 10.3% các ca nhập viện do tiêu chảy ở trẻ em, trong khi đó khoảng 25% trường hợp nguyên nhân gây bệnh không được phân lập (My Phan và cộng sự, số liệu chưa được công bố).
Liệu pháp bù nước bằng đường uống (Oral rehydration solution - ORS) vẫn đóng vai trò chính trong điều trị theo hướng dẫn năm 2005 của WHO trong điều trị và quản lý bệnh tiêu chảy cấp ở trẻ em. Hơn nữa, các kháng sinh cũng được khuyến cáo trong điều trị nhiễm trùng đường tiêu hóa nghiêm trọng, và thường xuyên được sử dụng cho trẻ em nhập viện tại thành phố Hồ Chí Minh với các bệnh nhiễm trùng cấp tính. Tại Việt Nam, việc sử dụng bổ sung men tiêu hóa (probiotic) cũng thông dụng trong điều trị tiêu chảy. Thuật ngữ probiotic được dùng ngày nay, được giới thiệu đầu tiên bởi Parker năm 1971, theo trích dẩn của Schrezenmeir và de Vrese nằm 2001, và được dùng để chỉ “các sinh vật và các chất đóng góp vào sự cân bằng hệ vi sinh đường ruột 5. Source text - English The 1000 Genomes Project (www.1000genomes.org) is a systematic attempt to sequence the genomes of at least a thousand people from around the world to create the most detailed and medically useful picture to date of human genetic variation. Initiated in January 2008, it can be regarded as an extension of the Human Genome Project. It involves major research institutions across the world, including the Wellcome Trust Sanger Institute UK, the Beijing Genomic Institute, Shenzhen, China the National Human Genome Research Institute (NHGRI), part of the National Institutes of Health (NIH) in the USA. Currently the largest sources of funding are the NIH, the Wellcome Trust and the Chinese Government. The results of the study will be publicly accessible to researchers worldwide.
One of the most important features of the human genome is that every individual has a different genome sequence. Even though the genomes of any 2 people are more than 99% the same, the small fraction of variation can help explain individual differences in disease susceptibility, response to drugs or reaction to environmental factors. Our current knowledge of the human genome is based on a very small sample of individuals [the Human Genome Project (http://www.genome.gov/10001691) and the HapMap project (http://www.hapmap.org/) ], and there is much to learn about how the genome sequence varies within the general population. Better understanding of human genome variation would be extremely valuable for many different areas of medical research. In the long term, such information will be of direct practical benefit for the treatment and prevention of disease, by helping to develop medical interventions that are optimally suited for the particular characteristics of the people and populations in which they will be used.

Bắt đầu từ tháng 1 năm 2008, đây có thể được xem như sự mở rộng của dự án Bộ Gen Người. Dự án có sự tham gia của nhiểu viện nghiên cứu lớn khắp thế giới, bao gồm Viện Wellcome Trust Sanger, Anh; viện Gen Bắc Kinh, Quảng Đông, Trung Quốc; Viện nghiên cứu bộ gen người quốc gia Trung Quốc (NHGRI); một phần Viện sức khỏe Quốc gia (NIH) của Mỹ.
