Summary
l The Economist Intelligence Unit forecasts that growth in the BRICs (Brazil, Russia, India, China) will continue to outpace that of the biggest economies in the developed world in the coming quarters. However, the outlook for the G3 (US, Japan, Germany) suggests that a double-dip recession is now a diminishing risk.
l The G3 is expected to avoid outright deflation as exceptionally loose monetary policy offsets downward pressure on prices from deleveraging and sluggish labour markets. The BRICs will experience higher inflation, as capacity constraints emerge in some sectors. In addition, the BRICs are more exposed to increased raw material costs, which have a higher weighting in their consumer price indices. Output
The four BRIC economies (Brazil, Russia, India, China) have consistently outperformed the G3 (US, Japan, Germany) over the past decade. The BRICs’ share of global GDP has risen from 9% in 2000 to an estimated 14% in 2010, on the basis of market exchange rates. Over the same period, the G3’s share of global GDP is estimated to have fallen from 37% to 30% on a purchasing power parity (PPP) basis and from 46% to 41% on a market exchange-rate basis.
We forecast that this trend will continue in the medium and long term. Growth in the US will be constrained by an ongoing process of deleveraging by the private sector, and by the withdrawal of fiscal stimulus. Real growth in the fourth quarter of 2010 is forecast to be 0.2% in quarter-on-quarter terms, with growth for 2011 as a whole expected to fall to 1.5% from 2.5% this year. Japan will confront fiscal and demographic challenges at home, but can rely on robust Chinese demand to continue to provide
a source of export growth, even taking into account a loss of competitiveness from a strong yen. We forecast fourth-quarter real GDP growth of 0.4%, and a weaker outlook for 2011, with annual growth slowing to 1.2%. Germany’s export sector will benefit from strong demand in emerging markets but domestic demand is forecast to remain subdued; consumer demand fell on a quarter-on-quarter basis every quarter from the first quarter of 2009 to the first quarter of 2010, and the recovery since has been muted. We expect a growth slowdown in 2011 as recent benefits from the stock cycle end; growth on a quarter-on-quarter basis in the final quarter of 2010 is expected to fall to 0.3%, and to continue falling in subsequent quarters until a quarter of zero growth in the fourth quarter of 2011.
China is forecast to continue to post strong growth, despite both subdued growth in its developed¬country export markets and some tightening of domestic policy to contain inflation and curb growth in bank lending. We estimate fourth-quarter growth of 1.6%, with growth in the first three quarters of 2011 forecast to average 2.4% before falling back through to the end of 2011 and beginning of 2012. The impact of the global financial crisis on the BRIC economies was uneven. India’s closed economy was relatively insulated and it contined to grow strongly. China’s large export sector suffered from the collapse in global demand, but the government responded with a massive programme to stimulate housing construction and investment in infrastructure. This succeeded in keeping growth high over
2009 as a whole. Brazil’s improved fundamentals meant that it only suffered a slight growth contraction, and the economy has rebounded strongly in 2010. Russia, however, experienced a steep decline in output; the credit crunch exposed a reliance by Russian banks and companies on external financing, while the collapse in oil prices put pressure on the public finances and the rouble.
China will remain the fastest growing of the BRICs next year, but we forecast that India will grow at a faster rate than China from 2012. This reflects more favourable demographic trends in India where
more than one-half of the population is under 25, in contrast to China’s rapidly ageing population. Over the long term, India has greater scope for catch-up as more jobs are created in the underdeveloped manufacturing sector. Looking at the economy’s more immediate prospects, we estimate fourth¬quarter growth of 8.9% on a year-on-year basis.
Following growth of 7.5% this year, the fastest rate in decades, Brazil’s economy is forecast to slow to a more sustainable rate of 4.5% in 2011, with growth split evenly between the quarters. Currency strength will be an obstacle for some sectors as Brazil attracts substantial amounts of foreign capital, in part to finance the development of large offshore oilfields. Russia is forecast to grow by around 4% on an annual basis in 2010 and 2011, and although growth in the final quarter of 2010 is forecast to
exceed 4.5% on a year-on-year basis, we have reduced our forecast for 2010 as a whole from 4.2% to 4% in light of indications that household demand has weakened. Oil prices at current levels of more than US$80/barrel will keep the current account in surplus and prevent fiscal stresses, but deficiencies in the business climate will deter investment.

In the G3, policies of inflation-targeting and floating exchange rates are well established and were deemed to be working well during the “Great Moderation” of the past decade (a period of sustained, non-inflationary growth). However, although consumer prices were stable and low, asset prices were rising rapidly. Bubbles developed and eventually burst, giving rise to the greatest financial crisis of the modern era. Changes in the conduct of monetary policy are in prospect. Central bankers will need to take asset prices into account when setting policy rates. However, in the US the Fed is currently seeking to boost asset prices and avert deflation through its programme of quantitative easing (QE), whereby it prints money to buy government bonds and other assets with a view to stimulating demand.
We forecast that these measures will help to avert outright price falls in the US in 2011, although quarterly average inflation is forecast to remain below 0.5% until the final quarter of next year. Further out, a domestic demand recovery should help to create inflationary pressure. In the longer term, there are questions about the exit strategy from QE and whether the Fed will be able to sell its portfolio of assets back to the market without creating severe dislocations in credit markets.
Japan has been flirting with deflation for much of the past decade. The current strength of the yen will tend to entrench deflationary pressures. The Bank of Japan (BOJ, the central bank) has responded with its own version of QE (equivalent to around US$60bn) as well as intervening in the foreign¬exchange market, selling yen, for the first time since 2004. These measures are expected to help Japan to avoid deflation in 2011-12, although consumer price inflation will struggle remain above 0.1% on
a quarter-on-quarter basis over the coming quarters, and is estimated to be flat in the final quarter of 2010.
China experienced very low or negative inflation for much of the past decade as it emerged as a world manufacturing power and added huge amounts of capacity. But it has been the exception among the BRICs. In India, large fiscal deficits have contributed to inflationary pressures. Both Russia and Brazil have been undergoing a gradual price stabilisation process after experiencing high
inflation in the 1990s.
We forecast that inflation rates in the BRIC countries will converge in 2011-12. In Russia and India, inflation is forecast to continue to trend lower following the price surges of recent years. Indian price inflation is estimated to have fallen to 1.5% quarter on quarter in the final quarter of 2010, and is forecast to bottom out at 1% in the first quarter of 2011 before rising throughout the year. In Russia’s case, price growth on a quarter-on-quarter basis will remain higher in the near-term, with averages of 2% and 3% in the fourth quarter of 2010 and first quarter of 2011 respectively, before falling back as oil prices fall and the strength of the rouble helps to bear down on prices. The forecast uptick in Chinese inflation reflects the strength of domestic demand, and the authorities are tightening policy to ensure that inflation does not spiral out of control. We expect year-on-year inflation of 5% in the fourth quarter of 2010. Further modest monetary tightening is in prospect in Brazil to keep inflation within the target range of the Banco Central do Brasil (BCB, the Central Bank).
Exchange rates
There have been a number of themes and cycles in foreign-exchange markets since the onset of the financial crisis. The US dollar has played the role of a safe haven at times of risk aversion, rising strongly against the euro and emerging-market and commodity currencies in the second half of 2008. Conversely, when markets are in risk-seeking mode, the dollar has been used as a funding currency for carry trades and has fallen. Anticipation of QE2 contributed to heavy selling of the dollar in the third quarter of 2010.
The yen has experienced a sustained rally since mid-2008, largely reflecting its status as a safe haven currency, but the appreciation is also a consequence of Japan’s positive current-account balance. This is creating problems for Japanese manufacturers, which are shifting more of their production offshore. We expect the dollar to recoup some of its losses against the euro in 2011 as debt problems in the euro zone periphery remain to the fore, damaging sentiment towards the single currency. The yen is forecast to remain strong in 2011-12 but is vulnerable to a sell-off in the medium term as Japan’s saving rate delines, raising questions over the financing of its huge public debt.
When inflation differentials are taken into account, the profiles of the key bilateral exchange rates change slightly, and, significantly, the renminbi has experienced a stronger appreciation than the nominal rate would suggest. Conversely, the yen’s appreciation is less marked when very low Japanese inflation is taken into account.
Interest rates
Money market rates in the G3 have fallen sharply in the wake of the crisis, reflecting cuts in policy rates to close to zero. We do not expect the US and the ECB to start raising policy rates until the second half of 2012. Money market rates will tick upward ahead of policy rates in response to market conditions, a trend already established in the euro zone.
Money market rates in the BRICs have been much higher and more volatile than in the developed world. This reflects higher inflation in some cases, plus a lack of credibility on the part of policymakers. For example, in Brazil, even though the Central Bank has built up a good track record on price stabilisation over the past decade, real interest rates have remained among the highest in the world. The government’s high borrowing needs is also a factor, exerting upward pressure on interest rates.
In Russia, rates spiked higher following the failure of Lehman Brothers in September 2008. This was a reversal of the trend in the other BRIC countries and the rest of the world. Russia raised interest rates and ran down its foreign-exchange reserves in an unsuccessful attempt to support the rouble, which came under pressure as oil prices collapsed. This contrasted with the stance of Brazil’s Central Bank which allowed the Real to depreciate and conserved its foreign-exchange reserves.

G3 policymakers responded to the financial crisis by slashing short-term rates to zero or close to zero. Risk aversion created a bid for the perceived risk-free asset class of government bonds. Even so, there was an inevitable steepening of the yield curve as declines in long-term bond yields failed to match the drastic declines at the front-end of the curve. The steepening yield curve provides a
politically acceptable way for banks—which typically borrow short and lend long—to recapitalise. We expect the Fed and the ECB to keep short-term rates at current levels until the second half of 2012. This underpins our forecast of elevated term spreads over the next two years.

Points to take away
l After a strong rebound from the 2008 recession, we forecast growth in the BRIC countries to moderate slightly, but to remain strong. The G3 economies face structural challenges, which will dampen growth prospects in the medium term, although the risk of a return to recession is now believed to have diminished.
l The two economic groupings face contrasting inflation challenges: for the BRICs, inflation prospects are vulnerable to commodity price fluctuations, while for the G3, falling prices remain a possibility. The threat of deflation will mean loose monetary conditions and the use of unconventional monetary policy in the G3 for the near term.
l Given underlying structural weaknesses in the economies of some euro member countries, we expect the euro to depreciate against the dollar over the coming year, although the outlook for exchange rates remains unusually uncertain. We also forecast a steady, moderate appreciation of the renminbi against the dollar in 2011 and 2012.

Nel corso dell’ultimo decennio, le quattro economie BRIC (Brasile, Russia, India, Cina) hanno notevolmente superato i G3 (Stati Uniti, Giappone, Germania). Sulla base dei tassi di cambio di mercato, la percentuale del PIL mondiale “coperta” dai BRIC è cresciuta dal 9% del 2000 fino a un probabile 14% nel 2010. Nello stesso periodo, si stima che la percentuale del PIL dei G3 sia caduta dal 37% al 30%, sulla base della parità di potere di acquisto (PPP) e dal 46% al 41% sulla base dei tassi di cambio di mercato.
Prevediamo che questo trend continuerà nel medio e nel lungo periodo. Negli Stati Uniti, la crescita sarà ridotta da un processo di deleveraging in corso nel settore privato e dal ritiro dello stimolo fiscale. Prevediamo che la crescita reale nel quarto trimestre del 2010 sarà dello 0.2% in termini di confronto fra trimestri, con una crescita complessiva per il 2011 prevista in calo fino all’1.5% dal 2.5% di quest’anno. Il Giappone dovrà affrontare delle sfide “interne” di natura fiscale e demografica, ma potrà continuare a poggiare sulla forza della domanda Cinese come traino per le sue esportazioni, anche tenendo in considerazione la perdita di competitività dovuta ad uno yen molto forte sui mercati valutari. Prevediamo, nel quarto trimestre, una crescita reale del PIL giapponese dello 0.4% e uno scenario di maggiore debolezza per il 2011, con una crescita del PIL annuale in rallentamento fino all’1.2%. Il settore delle esportazioni della Germania beneficerà della forte domanda nei mercati emergenti ma, prevediamo che la domanda interna rimanga “repressa”; la domanda al consumo, su base trimestrale, è caduta ogni trimestre dal primo trimestre del 2009 al primo trimestre del 2010 e, da allora, la ripresa è mutata. Ci aspettiamo un rallentamento della crescita nel 2011, essendo finiti i benefici recenti del ciclo delle scorte; ci aspettiamo che, nell’ultimo trimestre del 2010, la crescita su base trimestrale cada fino allo 0.3% e che continui a cadere nei trimestri successivi fino ad una crescita zero nell’ultimo trimestre del 2011.
Prevediamo che la Cina continui a segnare una crescita robusta, a dispetto sia della crescita “repressa” nei suoi mercati di esportazione delle nazioni sviluppate e un certo irrigidimento della politica economica domestica mirato a contenere l’inflazione e tagliare la crescita del prestito bancario. Stimiamo una crescita nel quarto trimestre dell’1.6%, con una crescita media nei primi tre trimestri del 2011 del 2.4%, prima di ricadere alla fine del 2011 e all’inizio del 2012. La crescita della Cina continuerà ad avere un effetto galvanizzante sugli altri mercati emergenti, compresi il Brasile e la Russia, grazie alla sua domanda di materie prime.

L’impatto della crisi finanziaria mondiale sulle economie BRIC è stato irregolare. L’economia Indiana, più chiusa, è rimasta relativamente isolata e ha continuato a crescere vigorosamente. L’ampio settore di esportazione Cinese ha pagato il crollo della domanda mondiale, ma il governo ha risposto con un programma massiccio di stimolo del settore edile e degli investimenti in infrastrutture. In questo modo è riuscito a mantenere alta la crescita per tutto il 2009. I migliorati fondamentali del Brasile hanno permesso di soffrire solo una leggera riduzione della crescita e, nel 2010, l’economia si è ripresa con vigore. La Russia, ad ogni modo, ha conosciuto un acuto declino della produzione; la contrazione creditizia ha costretto le banche e le aziende Russe a un finanziamento esterno, mentre il crollo dei prezzi del petrolio ha messo pressione sulle finanze pubbliche e sul rublo.
Il prossimo anno, la Cina rimarrà la nazione a più rapida crescita fra le nazioni BRIC, ma prevediamo che, dal 2012, l’India crescerà ad un passo maggiore della Cina. Questo è il riflesso di trend demografici Indiani più favorevoli, dove più della metà della popolazione è sotto i venticinque anni, rispetto ad una popolazione Cinese che invecchia più rapidamente. Nel lungo periodo, l’India ha uno spazio maggiore per creare più posti di lavoro nel sottosviluppato settore manifatturiero. Guardando alle più immediate prospettive dell’economia, stimiamo una crescita dell’8.9% nel quarto trimestre, su base annuale.
In conseguenza di una crescita del 7.5% (quest’anno), la crescita maggiore in decenni, prevediamo che, nel 2011, l’economia Brasiliana rallenti fino ad un tasso più sostenibile del 4.5%, con una crescita divisa uniformemente fra i trimestri. La forza della moneta rappresenterà un ostacolo per alcuni settori poiché il Brasile attrae significative quantità di capitale straniero, in parte per finanziare lo sviluppo di grandi campi petroliferi in mare aperto. Prevediamo che la Russia cresca di circa il 4% su base annuale nel 2010 e nel 2011 e, sebbene prevediamo che la crescita nell’ultimo trimestre del 2010 eccederà il 4.5% su base annuale, abbiamo rivisto al ribasso le nostre previsioni per il 2010 nel complesso da 4.2% a 4%, alla luce dell’indebolimento della domanda dei proprietari di case. I prezzi del petrolio ai livelli attuali di più di 80 dollari Statunitensi al barile manterrà in surplus l’attuale conto delle partite correnti in surplus e consentirà di prevenire degli irrigidimenti fiscali, ma la generale sfiducia nell’ambiente economico ridurrà gli investimenti.
I prezzi.
Nel gruppo dei paesi BRIC, l’inflazione da prezzi è stata notevolmente più alta di quella registrata nel mondo sviluppato. La formulazione della politica monetaria e dei tassi di cambio è meno “progredita” nei paesi BRIC che nei G3. Il maggiore peso dei beni alimentari e dell’energia negli indici dei prezzi al consumo nei mercati emergenti ha lasciato i BRIC più esposti rispetto ai G3 agli aumenti dei prezzi delle materie prime. Sebbene le economie BRIC abbiano conosciuto dei rapidi incrementi nei prezzi delle attività patrimoniali , invertitisi alla fine del 2008 e all’inizio del 2009, la correzione si è dimostrata meno dannosa rispetto a quanto avvenuto nei paesi sviluppati. Una ragione è da ritrovare nella migliore forma dei bilanci delle imprese e delle banche nei BRIC e nella maggiore robustezza dei settori bancari (con l’eccezione della Russia).

Nelle economie dei G3, le politiche di inflation-targeting e di tassi di cambio fluttuanti sono ben collaudate e sembra che abbiano avuto i loro effetti positivi durante la “Grande Moderazione”, “Great Moderation”, del decennio passato (un periodo di crescita sostenuta, non accompagnata da inflazione). Ad ogni modo, nonostante i prezzi al consumo siano rimasti stabili e bassi, i prezzi delle attività patrimoniali sono cresciuti rapidamente. Si sono formate delle bolle e alla fine sono scoppiate, lasciando la scena alla più grande crisi finanziaria dell’età moderna. Sono prossimi dei cambiamenti nella conduzione della politica monetaria. Nel fissare i tassi di politica economica, i banchieri centrali dovranno tenere in considerazione i prezzi delle attività patrimoniali. Negli Stati Uniti, la FED sta cercando di stimolare i prezzi delle attività patrimoniali ed evitare una deflazione attraverso un suo programma di Facilitazione Quantitativa (FQ-QE), con cui stampa danaro per acquistare obbligazioni di stato e altre attività patrimoniali con un occhio allo stimolo della domanda.
Prevediamo che, negli Stati Uniti e nel 2011, questi interventi aiuteranno ad evitare la rovinosa caduta dei prezzi, nonostante (prevediamo che) l’inflazione trimestrale media rimarrà sotto lo 0.5% fino all’ultimo trimestre del prossimo anno. Più in là, una ripresa della domanda domestica dovrebbe aiutare a creare una pressione inflazionistica. Nel lungo periodo, ci si interroga sulla strategia di uscita dalla F.Q. e se la Fed riuscirà a rimettere sul mercato il suo portafoglio di attività finanziarie e patrimoniali senza creare delle severe dislocazioni di risorse nei mercati del credito.
Per gran parte del passato decennio, il Giappone ha avuto un buon rapporto con la deflazione. L’attuale forza dello yen tenderà a trincerare delle pressioni deflazionistiche. La Banca del Giappone (BOJ, Bank of Japan) ha risposto con la sua versione della F.Q. (equivalente a circa 60 bilioni di dollari statunitensi) ed è intervenuta nel mercato dei cambi esteri, vendendo yen, per la prima volta dal 2004. Ci si aspetta che questi interventi aiutino il Giappone ad evitare la deflazione nel 2011-2012, nonostante l’inflazione dei prezzi al consumo rimarrà ostinatamente sopra lo 0.1% su base trimestrale nel corso dei prossimi trimestri e, si stima, si allenterà nell’ultimo trimestre del 2010.
Per gran parte dell’ultimo decennio, la Cina ha assistito a un’inflazione molto bassa o negativa. Si è trattato, tuttavia, di un’eccezione fra i paesi BRIC. In India, gli ampi deficit fiscali hanno contribuito a delle pressioni inflazionistiche. Sia la Russia, sia il Brasile hanno conosciuto un graduale processo di stabilizzazione dei prezzi dopo un elevato livello di inflazione negli anni 90.
Prevediamo che, nel biennio 2011-2012, i tassi d’inflazione nelle nazioni BRIC convergeranno. Prevediamo che in Russia e in India l’inflazione continui il trend verso il basso dopo gli aumenti dei prezzi degli ultimi anni. Si stima che l’inflazione dei prezzi Indiana sarà caduta fino all’1.5% su base trimestrale nell’ultimo trimestre del 2010 e prevediamo che raggiunga il punto minimo dell’ 1% nel primo trimestre del 2011 prima di crescere per il resto dell’anno. Nel caso della Russia, la crescita dei prezzi su base trimestrale rimarrà più alta nel brevissimo periodo, con medie del 2% e del 3%, rispettivamente nel quarto trimestre del 2010 e nel primo trimestre del 2011, prima di cadere all’indietro per la discesa dei prezzi del petrolio e la forza del rublo. Il previsto aumento dell’inflazione Cinese riflette la forza della domanda domestica e le autorità stanno rafforzando la politica economica per garantire che l’inflazione non vada a spirale e fuori controllo. Nel quarto trimestre del 2010, ci aspettiamo un’inflazione su base annuale del 5%. Ulteriori modesti irrigidimenti della politica monetaria sono attesi in Brasile per mantenere l’inflazione nell’ambito dell’obiettivo prefissato dal Banco Central do Brasil (BCB, la Banca Centrale Brasiliana).
Tassi di cambio
Dall’inizio della crisi finanziaria, c’è stato un certo numero di cicli nei mercati dei cambi esteri. Nella seconda metà del 2008, in tempi di avversione al rischio, il dollaro statunitense ha giocato un ruolo di paradiso di sicurezza, crescendo notevolmente rispetto all’euro e alle monete dei mercati emergenti e di riferimento delle materie prime. Viceversa, in periodi di inclinazione al rischio, il dollaro è stato usato “come valuta di conversione per l’industria dei trasporti ed è caduto”. L’anticipazione della seconda facilitazione quantitativa ha contribuito a pesanti vendite di dollari nel terzo trimestre del 2010.
Dalla metà del 2008, lo yen ha conosciuto un sostenuto recupero, riflettendo ampia-mente il suo status di moneta di sicurezza, ma l’apprezzamento è anche una conse-guenza del positivo bilancio delle partite correnti Giapponesi. Questo sta creando problemi ai produttori Giapponesi, che stanno spostando gran parte della propria produzione all’estero. I responsabili della politica economia hanno cercato di frenare la forza dello yen intervenendo nel mercato dei cambi, per la prima volta dal 2004, e introducendo un programma di Facilitazione Quantitativa relativamente ridotto.

Nel 2011, ci aspettiamo che il dollaro recuperi parte delle perdite nei confronti dell’euro in quanto i problemi del debito nella periferia di area euro rimangono in primo piano, danneggiando la fiducia nella moneta unica. Nel 2011-12, lo yen dovrebbe rimanere forte ma è vulnerabile ad una svendita nel medio termine determinata da un declino dei tassi di risparmio Giapponesi, sollevando degli interrogativi sul finanziamento dell’enorme debito pubblico.
Quando si prendono in considerazione i differenziali d’inflazione, i profili dei tassi di cambio bilaterali chiave cambiano leggermente e, in modo significativo, lo yuan ha conosciuto un più forte apprezzamento rispetto a quello che suggerirebbe il tasso nominale. Viceversa, l’apprezzamento dello yen è meno marcato quando viene preso in considerazione il basso livello dell’inflazione Giapponese.
I tassi di interesse.
Nei G3, sull’onda della crisi, i tassi di mercato monetario sono notevolmente caduti, come riflesso dei tagli dei tassi d’interesse vicini allo zero. Ci aspettiamo che gli Stati Uniti e la BCE comincino ad aumentare i tassi d’interesse solo dopo la seconda metà del 2012. I tassi del mercato monetario andranno verso l’alto in anticipo rispetto ai tassi d’interesse (nominali) come risposta alle condizioni di mercato, un trend già consolidato in area euro.
Nei BRIC, i tassi del mercato monetario sono stati molto più alti e più volatili che nel mondo sviluppato. Questo riflette un’inflazione più elevata in alcuni casi, più una mancanza di affidabilità nei confronti dei responsabili della politica economica. In Brasile, per esempio, anche se la Banca Centrale ha costruito una buona striscia di record nel corso dell’ultimo decennio sulla stabilizzazione dei prezzi, i tassi di interesse reali sono rimasti fra i più alti nel mondo. Anche le forti esigenze di finanziamento del governo hanno contribuito ad esercitare una pressione al rialzo dei tassi d’interesse.
In Russia, nel settembre del 2008, a seguito del fallimento di Lehman Brothers, i tassi d’interesse hanno segnato un picco verso l’alto. Si è trattato di un’inversione del trend osservato nelle altre nazioni BRIC e nel resto del mondo. La Russia ha aumentato i tassi di interesse e ha liberato le sue riserve di monete straniere in un tentativo comunque infruttuoso di sostenere il rublo, che erano andato sotto pressione con il crollo dei prezzi del petrolio. Questo ha contrastato con la posizione della Banca Centrale del Brasile che ha permesso al Real di deprezzarsi e ha conservato le sue riserve in valute straniere.
Ci aspettiamo che nei BRIC, i tassi del mercato monetario salgano gradual-mente nel 2011-12 poiché la robusta domanda crea delle pressioni inflazionistiche.

I responsabili della politica economica dei G3 hanno risposto alla crisi finanziaria tagliando i tassi d’interesse di breve termine portandoli a zero o vicini allo zero. L’avversione al rischio ha creato un’offerta per le obbligazioni di stato percepite come non rischiose. Anche in questo modo, c’è stata un’inevitabile impennata della curva dei rendimenti poiché i cali dei rendimenti delle obbligazioni di stato a lungo termine hanno fallito nel pareggiare i drastici cali alla fine della curva. Punti da evidenziare.
• Dopo una forte ripresa rispetto alla recessione del 2008, prevediamo che la crescita nelle nazioni BRIC si modererà leggermente, ma rimarrà vigorosa. The topic of my talk, the aquaporins, is something that we have been studying in our lab for the last 15 years. It is not something that I anticipated when starting my career. In fact, I am a hema- tologist, but in science, sometimes what we find is not what we are looking for. A typical middle-aged man, I weigh 75 kg, of which 50 are water. This is something that we all share. Water is the major component of all our cells in all of our tissues. This is also shared with all other life forms, other vertebrates, invertebrates, microorganisms, and plants. Water can be de- scribed as the “solvent of life”—without water, there is no life. Multicellular life comes with potential problems. One of these problems is the movement of fluids across biological barriers, and the principal barrier for most of our tissues is the plasma membrane. The aquaporins, which I will describe, are an answer to how water crosses biological membranes, but specific ques- tions still need be defined. I am very pleased to be speaking to the American Thoracic Society because I believe that many of the questions related to fluid movements in lung are still unsettled.
When we began our studies some years back, the problem
of membrane water permeability had already been looked at by a generation of biophysicists and physiologists. With the discov- ery of the lipid bilayer in the 1920s, it was speculated that a finite degree of water permeability would occur by simple diffusion through the plasma membrane. However, the work of very vigor- ous biophysicists and physiologists indicated that in certain tissues—for example, renal tubule, secretory glands, and red cells—the water permeability is much larger than could be ex- plained through simple diffusion through the plasma membrane. And these investigators predicted correctly that there must be specialized water-selective channels in these membranes. The current view is that both mechanisms occur—diffusion through all membranes and flow through aquaporins that are present in certain special membranes. The biophysical differences are quite significant; diffusion is a low-capacity bidirectional movement of water, whereas water channels have a high capacity and great selectivity for water. The channels are so selective that water passes through them, and acid does not. As we all know, protons exist in fluid as the hydronium ion. This distinction is really quite important. Every day, our kidneys filter and reabsorb about
180 L of water. If we do not reabsorb that water, we would die
of dehydration. If we reabsorb the water, along with protons, we would become systemically acidotic. The movement of water through the aquaporins is driven by osmotic gradients. For exam- ple, red cells dropped into seawater will shrink because water leaves the cells; red cells dropped into fresh water will swell and explode since water enters the cell. The process of osmosis that

we all learned about as children is known to occur very rapidly in membranes in which aquaporins are present.
An experiment by Bob Macy at the University of California, Berkeley, in 1970 made a very important observation. It was known that diffusion of water through membranes is not inhib- ited by any known pharmacologic agents. Macy discovered that mercuric chloride reduces the water permeability of red cells, and when the membranes have been treated with reducing agents, the water permeability was restored. Macy concluded that the water channel protein must contain a free sulfhydryl somewhere within the pore. This provided very strong evidence that water channels must exist; however, most physiologists and other scientists remained very skeptical. They could not identify water channel molecules, much less purify them, clone them, express them, or reconstitute them.
Our laboratory got into the water channel field by accident. We discovered a polypeptide in red cells that we didn’t expect to see. Interestingly, it was identical to a polypeptide in the kidney. So, on a great leap of faith, we decided to clone the complementary DNA (cDNA). This was in the early days before the genome was sequenced, so it was a lot of hard work. We obtained a cDNA encoding a 269–amino acid polypeptide. Iden- tical transcripts were obtained from red cell and kidney libraries. The new protein was found to have six bilayer-spanning domains. The N-terminus of the protein resides within the cell, and then two interesting repeats appear, each corresponding to three bilayer-spanning domains. They are not a perfect sequence re- peats but are genetically similar sequences. Most curiously, the two repeats appear to be oriented at 180° to each other. We looked at the DNA database in 1991. We found a number of similar sequences had been reported, but none of the proteins had a function that had been defined. Thus, we obtained no obvious clue to the function of our protein. The related se- quences corresponded to a protein from the lens of eye from beef cattle, a protein from brain of Drosophila fruit flies, a protein that permits Escherichia coli to use glycerol as a carbon source, and a series of related proteins from various plant tissues. It was really the observation that red cells and kidney tubules are highly water permeable, and the existence of plant homologs, that caused us to pursue this further.
We established the water transport function of our protein in collaboration with our colleague Bill Guggino in the Physiology Department at Johns Hopkins. To do this, we injected the com- plementary RNA into Xenopus laevis oocytes. These amphibian eggs are about a millimeter in diameter and are known to be quite water impermeable (Figure 1). On the left is a controlled oocyte injected only with buffer alone and on the right is an oocyte injected with the complementary RNA for our new pro- tein. After culture, the oocytes looked about the same. But when they were then dropped into distilled water, an amazing difference was observed. The controlled oocyte had failed to swell, the test oocyte had swollen and exploded. This produced much jubilation in the laboratory! Shown in Figure 1 is Gregory Preston, the Postdoctoral Fellow who did these studies. I took this photograph of Greg three years after the discovery, and he was still celebrating. So, I think that young scientists need to know that gratification is often delayed in science, and it is even permissible to cheer for yourself.
We spent a considerable amount of effort studying the struc- ture of our new water channel protein. The internal repeats contain two loops B and E, which join the second and third, then the fifth and sixth bilayer-spanning domains and contain a

degree of hydrophobicity. We thought that loops B and E may actually fold together and make a central aqueous pore. There is a special motif (Asn-Pro-Ala [NPA]) in each loop that overlaps in the center of the pore. The original hourglass model was just a pencil-and-paper sketch. But in fact it has been confirmed by high-resolution protein studies that we undertook with Professor Yoshinori Fujiyoshi from Kyoto, Japan, and Professor Andreas Engel from the Biocenter in Basel, Switzerland.
Together with these scientists, we solved the structure of the highly purified AQP1 protein from our lab. The reconstituted protein was 100% water permeable, indicating that the structure that we would deduce would be the biologically relevant struc-
ture. By a combination of cryo-electron microscopy and atomic force microscopy, a three-dimensional electron density map at
3.8-A˚ resolution was calculated, and models were built. The protein is tetromeric, but the functional unit is the monomer. A single aqueous pore passes down through the center of the
protein. The protein fold has been further defined by X-ray photographic studies from Bob Stroud from the University of California, San Francisco, and Bing Jap from Lawrence Berkeley Laboratory. Molecular dynamic simulations were undertaken by deGroot and Grubmu¨ ller in Go¨ ttingen and Schulten and colleagues at the University of Illinois at Urbana- Champaign.
We now have a clear understanding of how the aquaporin proteins work. Aquaporins have a narrow pathway that is a very tight fit for water, the smallest biological molecule (Figure 2).
Shown in Figure 2 is a space-filling representation of the single water molecule in the very center of the pore of the cross-section,

and you can see that the adjacent charged residues, arginine and histidine, are very close. One can see here that there is a perfectly conserved residue, a cysteine, which is the site of mercurial inhibition. Now, how did this water pass through the membrane, but fail to carry protons? My father was a chemistry professor back in Minnesota, and he liked to tell his first-year students about the chemistry of water—H2O. Because it is 18 Da in mass, the molecule would be predicted to be a gas, which of course water is in the atmosphere. But in solution, the hydrogen bonds between water molecules cause water to be a liquid. In bulk solution, the water molecules are close together and hydrogen bonding occurs. This allows free movement of protons hopping between the molecules. In the extracellular vestibule of the hour- glass and in the intracellular vestibule, water exists in bulk solu-
tion (Figure 3). But the center of the aquaporin has a 20-A˚ trim
span where water transits the pore in single file. The narrowest diameter of the pores is 2.8 A˚ —just big enough for a single water molecule. A fixed positive charge on the adjacent arginine side chain will repel protons. The water molecules then are spaced within the pore at intervals so that hydrogen bonding cannot occur between them. A second barrier exists in the center
of the pore, where an isolated water molecule will transiently form hydrogen bonds to the side chains of two highly conserved asparagines residues. This provides a very interesting mecha- nism—one that allows water to move with no resistance.
We and others sought by homology cloning to identify other members of the aquaporin protein family. A gene pileup repre- sents a partial listing of human aquaporins (Figure 4). We now know of 12 human sequences that are closely related to AQP1. They are classed into two subsets: those permeated by water alone, the classical aquaporins, and those permeated by water plus glycerol, the aquaglyceroporins. Knowing the biophysical functions of the proteins and the sites where they are expressed permits us to predict their physiologic and pathophysiologic functions. I will briefly discuss some of each subset. AQP1 in the kidney is responsible for the countercurrent concentrating mechanism in the proximal tubules. AQP2 is the vasopressin- regulated water channel in the collecting duct. Mutations in the gene encoding AQP1 cause mild nephrogenic diabetes insip- idus, but mutations in the gene encoding AQP2 are much more severe. AQP4 is expressed in brain, and is involved in both the pathogenesis and the recovery from brain edema. AQP0 is expressed in lens, and mutations in this protein cause cataracts. The aquaglyceroporin AQP7 is expressed in fat tissue where it is responsible for glycerol release during fasting and starva- tion, while AQP9 in liver permits glycerol uptake by liver for gluconeogenesis.
To define the physiologic functions of AQP1, we collaborated with Søren Nielsen from the University of Aarhus in Denmark who is a world-class microscopist. Søren localized the AQP1 in lung and other tissues as well, but lung is the topic of the talk today. AQP1 is present in the capillary endothelium in a peri- bronchiolar distribution. In the alveolar section, a lower degree of specific staining is visible (Figure 5). Immunogold electromi- croscopy of a single capillary endothelial cell from human lung exhibits immunogold labeling of both the lumenal membrane and the ablumenal membrane of the capillary (Figure 6). Studies by our colleague Landon King in the Division of Respiratory and Critical Care Medicine at Johns Hopkins demonstrated that the level of AQP1 expression in lung microvasculature is not constant. The amount of AQP1 rises dramatically in lung at the time of birth. In addition, AQP1 expression is exquisitely sensitive to corticosteroids—about an eightfold increase in ex- pression in adult rat lung (Figure 7, left panel). Like many others, I suffer from a mild form of asthma; I use a corticosteroid inhaler,
and the beneficial effects may be partly explained by the induc- tion of AQP1 expression.
AQP1 protein expression can be induced, but expression
levels can also be reduced, because the protein is degraded by the ubiquitin pathway. Here I should add that my talk is going to lead up to the one by Aaron Ciechanover, discoverer of the ubiquitin pathway (see page 21). In another study, Landon King looked at the degradation of AQP1 by the ubiquitin proteosome in cultured fibroblasts (Figure 7, right panel). Fibroblasts nor- mally only express very low levels of AQP1 when cultured in isotonic medium. Immunoprecipitations analyzed by antiubiqui- tin immunoblotting show that the AQP1 is polyubiquitinated—a low level of AQP1 is expressed and rapidly ubiquitinated tar- geting the protein for degradation. When fibroblasts are grown in culture medium made hypertonic by the addition of 200 mOsm sorbitol, the protein accumulated in abundance. Immunoprecipi- tations analyzed by antiubiquitin immunoblotting show that when fibroblasts are grown in hypertonic media, AQP1 is not ubiquitinated, and the level of protein rises dramatically. This provides an efficient mechanism for effectively accumulating AQP1 protein when it is needed and rapidly reducing AQP1 protein when no longer needed by the cell.
The lung is complicated, and Søren Nielsen and Landon King have defined the presence of multiple aquaporins at different levels of the respiratory tree (Figure 8). As described, AQP1 is present in capillaries and fibroblasts below the basement mem- brane. A different expression pattern is seen in the surface epi- thelium that lies above the basement membrane. Surface colum- nar cells contain AQP4 along the basolateral membranes. The adjacent goblet cells apparently do not contain aquaporins. The basal cells that reside along the basement membrane do not reach the surface and express high levels of AQP3. Thus, a clear distinction exists at the sites where AQP1, AQP3, and AQP4 are expressed. Still more complexity is found in the submucosal glands where AQP5 is expressed.
How does one distinguish the importance of multiple aquap- orin proteins that exist alongside each other in the complex airway epithelia? The level of expression is tightly regulated
during fetal development. AQP1 and AQP3 are expressed be- fore birth, but AQP4 is only expressed after birth. So here is an example of where clinical medicine provides basic scientific insight. Mice with gene knockouts have been prepared and have provided important insight. But as physicians, we are really inter- ested in the human biology.
The structural gene for AQP1 is colocalized with a specific blood antigen on human chromosome 7. As a hematologist, I am interested in blood group antigens. Although there are hundreds of different antigens, they comprise about 25 families—
each family being a genetic locus. As it turns out, the Colton blood antigen (Co) is a simple polymorphism on the surface of the AQP1 protein. Most of us have the same Co blood types, so they are usually not important clinically. Nevertheless, our colleagues at the International Blood Group Referencing Labo- ratory in Bristol, England, have only identified a handful of individuals lacking Co. So of course these are the very people that we became interested in. The Co-null individuals we were able to study were all women who apparently became sensitized during pregnancy. We determined that each was homozygous for different disrupting mutations in the gene encoding AQP1—a frameshift, an exon deletion, or a structurally important missense mutation—and were AQP1-null. Our Johns Hopkins colleague Landon King studied two unrelated AQP1-null individuals. A longstanding and distinguished member of the ATS, Landon was an ideal scientist to evaluate AQP1 in lung. The AQP1- null individuals have circulating anti-Co antibodies and are un- able to tolerate a blood transfusion from other blood donors.

Nevertheless, the AQP1-null individuals look and feel well. But clinical medicine provides us with a very interesting and powerful approach to identifying the importance of proteins. Presumably, many of us bear mutations in different genes. While these muta- tions usually do not cause symptoms, if we’re properly stressed, they can provoke a disease phenotype.
Landon King, in collaboration with Robert Brown at Johns
Hopkins, used high-resolution computed tomography scans of the lung to evaluate lung capillary water permeability of these individuals before and after intravenous infusion of saline. Shown in Figure 9 are images from normal individuals: a 2-mm bronchiole and an adjacent venule at baseline and the same structures after infusion of 3 L of physiologic saline. It is obvious that the venule has become distended after intravenous fluid caused engorgement of the blood microvasculature. Similar in- creases are found in all normal individuals and all of the AQP1- null individuals. After infusion of saline, the wall thickness of the bronchioles in all normal individuals has increased due to water released from the blood vessels into the surrounding soft tissues. Thus, the bronchiolar wall thickness has increased as water collects in the perivascular soft tissues, forming incipient pulmonary edema. Surprisingly, the AQP1-null individuals had no such release of fluid to the surrounding soft tissues. Their
bronchioles remained unchanged throughout the study—strong evidence that they have clear and measurable decrease in the vascular water permeability. At this point, we can only speculate about the clinical significance of this finding. With the first breath of life, the infant’s lung absorbs amniotic fluid through the airway epithelium, and this is taken into the surrounding blood vessels. Thus, AQP1 may be important in perinatal fluid absorption by lung. If this is a reason for the low prevalence of the AQP1- null state, then presumably those who survive are somehow compensating for the lack of AQP1 in the microvasculature.
Now, I will talk about another aquaporin from the lungs: AQP5 is expressed in secretory glands, including airway submu-
cosal glands, lacrimal glands, salivary glands, and sweat glands. AQP5 is expressed in the apical membrane of the secretory glands (Figure 10). The apical membrane where AQP5 resides is the last membrane water passes through during the secretion of airway fluids, tears, saliva, and sweat. Evidence suggests that this is important in clinical medicine. One study undertaken in collaboration with Chris Delporte in Brussels and a second study undertaken in collaboration with Kazuo Tsubota in Tokyo ana- lyzed biopsies from a small number of patients with Sjo¨ gren’s syndrome. This well-recognized disorder leads to dry eye and dry mouth problems, and pulmonary disorders as well. While

Sjo¨ gren’s syndrome is known to be an autoimmune problem, loss of glandular function often occurs in glandular elements where infiltration of lymphocytes is not seen. Normal biopsies have intense anti-AQP5 staining at the apical surface (Figure 11, left panel). In contrast, diffuse presence of AQP5 protein is notable throughout the glandular epithelium (Figure 11, center panel). This indicates that a mistake in trafficking of AQP5 protein has occurred, presumably as a secondary defect. An independent study of a larger number of Sjo¨ gren’s syndrome biopsies was undertaken by Roland Jonsson, from the University of Bergen in Norway, and his collaborators. Interestingly, these investigators found normal AQP5 distribution, but they deter- mined that AQP1 expression was reduced in the surrounding myoepithelial cells surrounding the glandular acini. So while there appears to be heterogeneity, all three groups found some aquaporin defects in this important clinical disorder, Sjo¨ gren’s syndrome, and we can as yet only speculate what is happening in submucosal glands of lung.
Another important role of AQP5 is in sweat glands. In collab- oration with Lene Nejsum and Søren Nielsen, analysis of sweat glands was undertaken comparing normal mice with AQP5 gene
knockout mice engineered by Anil Menon and Carissa Krane from the University of Cincinnati. Although both mice have normal-appearing sweat glands, the AQP5-null mice exhibit di- minished function of sweat glands after pharmacologic stimula-

Infatti, sono un ematologo, ma nella scienza, talvolta, ciò che scopriamo non è proprio quello che stiamo cercando. Tipico uomo di mezza età, peso 75 kg, di cui 50 sono acqua. È qualcosa che noi tutti condividiamo. L'acqua è l’elemento principale di tutte le cellule in tutti i nostri tessuti. Essa è anche condivisa con tutte le altre forme di vita, altri vertebrati, invertebrati, microrganismi e piante. L'acqua può essere descritta come "solvente della vita", senza acqua, non c'è vita. La vita multicellulare presenta diversi potenziali problemi. Uno di questi è il movimento dei fluidi attraverso le barriere biologiche e quella principale per la maggior parte dei nostri tessuti è la membrana plasmatica. Le acquaporine, che descriverò, sono una risposta a come l'acqua attraversi le membrane biologiche, ma c’è ancora molto da capire. Ogni giorno, i nostri reni filtrano e riassorbono circa 180 litri di acqua. Se non riassorbissimo quell''acqua, moriremmo di disidratazione. Se riassorbissimo l'acqua, insieme con i protoni, saremmo diventati sistemicamente acidotici. Il movimento dell'acqua attraverso le acquaporine è guidato da gradienti osmotici. Per esempio, i globuli rossi immersi in acqua di mare si ridurrebbero perché l'acqua esce dalle cellule; i globuli rossi immersi in acqua dolce si gonfierebbero ed esploderebbero poiché l'acqua entra nella cellula. abbiamo tutti imparato a conoscere da bambini si verifica molto rapidamente nelle membrane in cui sono presenti acquaporine.
Un esperimento di Bob Macy presso la University of California, Berkeley, nel 1970, ha fatto un'osservazione molto importante. Si sapeva che la diffusione di acqua attraverso le membrane non è inibita da agenti farmacologici conosciuti. Macy ha scoperto che il cloruro mercurico riduce la permeabilità all'acqua dei globuli rossi e quando le membrane sono state trattate con agenti riducenti, la permeabilità all'acqua è stata ripristinata. Era identico a un polipeptide del rene. Così, con un grande slancio di fiducia, abbiamo deciso di clonare il DNA complementare (cDNA). Questo è avvenuto nei giorni iniziali, prima che il genoma fosse sequenziato: è stato un gran, duro lavoro. Abbiamo ottenuto un cDNA che codifica un polipeptide a 269 aminoacidi. Identiche trascrizioni sono state ottenute da globuli rossi e librerie renali. La nuova proteina è stata trovata per avere sei domini a estensione bistrato. L'N-terminale della proteina risiede all'interno della cella e quindi si verificano due ripetizioni interessanti, ciascuna corrispondente a tre domini bistrato. Non sono ripetizioni a sequenza perfetta, ma sono sequenze geneticamente simili. Molto curiosamente, le due ripetizioni sembrano essere orientate a 180 l'una rispetto all'altra. Abbiamo esaminato il database del DNA nel 1991. Abbiamo scoperto che sono state riportate sequenze simili, ma nessuna delle proteine ha una funzione che era stata definita. Così, non abbiamo ottenuto alcun indizio evidente della funzione della nostra proteina. La relativa sequenza corrispondeva a una proteina dalla lente dell'occhio di bovini, una proteina dal cervello di moscerini Drosophila, una proteina che consente all’Escherichia coli di utilizzare glicerolo come fonte di carbonio e una serie di proteine da diversi tessuti vegetali. È stata proprio l'osservazione che i globuli rossi e i tubuli renali sono molto permeabili all'acqua e che esistono dei vegetali omologhi, che ci ha spinti ad andare avanti.
Abbiamo stabilito la funzione di trasporto dell'acqua della nostra proteina in collaborazione con il nostro collega Bill Guggino presso il Dipartimento di Fisiologia della Johns Hopkins. Per far questo, abbiamo iniettato le complementari RNA in ovociti.Xenopus laevis Queste uova anfibie sono di circa un millimetro di diametro e sono note per essere abbastanza impermeabili all'acqua (Figura 1). Sulla sinistra v'è un ovocita controllato iniettato solo con un tampone e sulla destra un ovocita iniettato con l'RNA complementare per la nostra nuova proteina. Dopo la coltura, gli ovociti sembravano gli stessi. Quando, tuttavia, sono stati immersi in acqua distillata, è stata osservata una differenza incredibile. certo grado d’idrofobicità. Abbiamo pensato che gli anelli B e E possono effettivamente incrociarsi e formare un poro centrale acquoso. Vi è un elemento speciale (Asn-Pro-Ala [NPA]) in ciascun anello che si sovrappone al centro del poro. Il modello a clessidra originale era solo uno schizzo carta e penna. In realtà è stato confermato da studi ad alta risoluzione della proteina che abbiamo eseguito con il Prof. Yoshinori Fujiyoshi da Kyoto, in Giappone, e il professor Andreas Engel dal Biocenter a Basilea, Svizzera.
Insieme a questi scienziati, abbiamo capito la struttura della proteina molto purificata AQP1 dal nostro laboratorio. La proteina ricostituita era permeabile all'acqua al 100%, indicando che la struttura che se ne deduce sarebbe la struttura biologicamente rilevante.
Con una combinazione di un microscopio crioelettronico e di un microscopio a forza atomica, è stata calcolata una mappa tridimensionale della densità di elettroni a risoluzione 3.8-A° e sono stati costruiti dei modelli. La proteina è tetromerica, ma l'unità funzionale è il monomero. Un singolo poro acquoso passa in basso attraverso il centro della proteina. L’ansa della proteina è stata ulteriormente definita da studi fotografici di Bob Stroud della University of California, San Francisco e Bing Jap del Lawrence Berkeley Laboratory. Simulazioni di dinamica molecolare sono state eseguite da DeGroot e Grubmu ¨ ller in Go ¨ Göttingen e Schulten e dai colleghi della University of Illinois a Urbana-Champaign.
Ora abbiamo una chiara comprensione di come lavorino le proteine di acquaporina. Le acquaporine presentano un percorso stretto che è una misura molto ridotta per l'acqua, la più piccola molecola biologica (Figura 2).

E si può vedere che i residui di carica adiacenti, arginina e istidina, sono molto vicini. Si può vedere che c'è un residuo perfettamente conservato, una cisteina, che è il sito d’inibizione mercuriale. Ora, come ha fatto quest'acqua ad attraversare la membrana, ma non riesce a trasportare protoni? Mio padre era professore di chimica in Minnesota e amava parlare ai suoi studenti del primo anno della chimica dell’acqua. Poiché ha una massa di 18Da, la molecola dovrebbe essere un gas che naturalmente è acqua nell'atmosfera. In soluzione, tuttavia, i legami idrogeno tra le molecole di acqua determinano la formazione dell'acqua liquida. In una soluzione “isolata” (bulk), le molecole d'acqua sono vicine e si forma il legame idrogeno. Questo permette la libera circolazione dei protoni saltellanti tra le molecole. Nel vestibolo extracellulare della clessidra e nel vestibolo intracellulare, l'acqua esiste in soluzione isolata (Figura 3). Il centro dell’acquaporina, tuttavia, ha un diametro di 20-A° dove le molecole d’acqua attraversano il poro in fila indiana. Il più stretto diametro dei pori è di 2,8 A ˚-appena abbastanza grande per una singola molecola d'acqua. Una carica fissa positiva sulla catena laterale di arginina respingerà i protoni. Le molecole di acqua quindi sono distanziate nei pori a intervalli in modo tale che il legame idrogeno non possa formarsi fra gli stessi. Una seconda barriera si ha nel centro del poro, dove una molecola di acqua isolata formerà transitoriamente dei legami idrogeno per le catene laterali di due residui di asparagina molto conservati. Questo fornisce un meccanismo molto interessante- che permette all'acqua di muoversi senza resistenza.
Noi e gli altri abbiamo cercato con clonazione omologa di identificare gli altri membri della famiglia di aquaporine. “Una collisione di gene” rappresenta un elenco parziale di acquaporine umane (Figura 4). Ora sappiamo di 12 sequenze umane che sono strettamente correlate a AQP1. Esse sono classificate in due sottogruppi: quelle permeate di sola acqua, le acquaporine classiche, e quelle permeate da acqua più glicerolo, le acquagliceroporine. Conoscere le funzioni biofisiche delle proteine e dei siti dove si esprimono, ci permette di prevedere le loro funzioni fisiologiche e fisiopatologiche. Io discuterò brevemente alcuni di questi sottoinsiemi. L’AQP1 nel rene è responsabile del meccanismo di concentrazione controcorrente nei tubuli prossimali. L’AQP2 è il canale d'acqua vasopressina-regolato nel condotto collettore renale. Le mutazioni nel gene che codifica l'AQP1 è causa di diabete insipido nefrogenico medio, ma le mutazioni nel gene che codifica AQP2 sono molto più gravi. L'AQP4 si ritrova nel cervello ed è coinvolta sia nella patogenesi sia nella riabilitazione da edema cerebrale. L'AQP0 si ritrova nel cristallino e le mutazioni di questa proteina causano cataratta. L'acquagliceroporina AQP7, si ritrova nel tessuto adiposo, dove è responsabile del rilascio del glicerolo durante il digiuno e la fame, mentre l'AQP9 nel fegato permette l'assorbimento del glicerolo dal fegato per la gluconeogenesi.
Per definire le funzioni fisiologiche dell'AQP1, abbiamo collaborato con Søren Nielsen dell'Università di Aarhus in Danimarca, che è un microscopista di fama mondiale. Søren ha localizzato l'AQP1 nei polmoni e in altri tessuti, ma il polmone è il tema di oggi. L'AQP1 è presente nell’endotelio capillare in una distribuzione peri-bronchiolare. Nella sezione alveolare, è visibile un minor grado di colorazione specifica (Figura 5). La microscopia immuno-elettronica di una singola cellula endoteliale capillare di polmoni umani evidenzia un’etichettatura immunogold sia della membrana lumenale sia della membrana ablumenale del capillare (Figura 6). Gli studi di un nostro collega della Divisione di Medicina respiratoria e Medicina della Landon King presso la Johns Hopkins hanno dimostrato che il livello di espressione di AQP1 nel microcircolo polmonare non è costante. La quantità di AQP1 aumenta drammaticamente nel polmone al momento della nascita. Inoltre, l'espressione AQP1 è squisitamente sensibile ai corticosteroidi- un aumento di otto volte nella sua espressione nel polmone del ratto adulto (Figura 7, riquadro di sinistra). Come molti, soffro di una lieve forma di asma; uso un inalatore di corticosteroidi e gli effetti benefici possono essere in parte spiegati dall'induzione di AQP1.
L'espressione della proteina dell'AQP1 può essere anche indotta, ma i livelli di espressione non possono essere ridotti anch'essi, perché la proteina è degradata da una via ubiquitina. A questo punto, devo aggiungere che il mio discorso sta per portare a quello di Aaron Ciechanover, scopritore della via ubiquitina. In un altro studio, Landon King ha osservato la degradazione dell'AQP1 dal proteosoma ubiquitino nei fibroblasti in coltura (Figura 7, riquadro di destra ). I fibroblasti, normalmente, esprimono solo livelli molto bassi di AQP1 quando sono coltivati in ambiente isotonico. Immunoprecipitazioni analizzate da immunoblotting antiubiquitine mostrano che l'AQP1 è poliubiquitinato- si esprime e si determina rapidamente un basso livello di AQP1, ottenendo la proteina per la degradazione. Quando i fibroblasti vengono coltivati in colture rese ipertoniche mediante l'aggiunta di 200 mOsm di sorbitolo, la proteina viene accumulata in abbondanza. Le immunoprecipitazioni analizzate mediante immunoblotting antiubiquitina mostrano che quando i fibroblasti sono coltivati in ambienti ipertonici, AQP1 non è ubiquitinato e il livello della proteina aumenta drammaticamente. Questo fornisce un meccanismo efficiente per accumulare efficacemente la proteina AQP1 quando è necessario e riducendo rapidamente la proteina AQP1quando non è più necessaria per la cellula.
Il polmone è complicato e Søren Nielsen e Landon King hanno definito la presenza di acquaporine multiple a diversi livelli dell'albero respiratorio (Figura 8). Come descritto, l'AQP1 è presente nei capillari e nei fibroblasti sotto la membrana basale. Un modello di espressione diverso è visto nella superficie epiteliale che si trova sopra la membrana basale. Le cellule colonnari di superficie contengono l'AQP4 lungo le membrane basolaterali. Apparentemente, le cellule caliciformi adiacenti non contengono acquaporine. Le cellule basali che si trovano lungo la membrana basale non raggiungono la superficie ed esprimono elevati livelli di AQP3. Così, si determina una chiara distinzione nei siti in cui si esprimono AQP1, AQP3 e AQP4. Una maggiore complessità si ritrova nelle ghiandole della sottomucosa, dove si esprime l'AQP5.
Come si fa a distinguere l'importanza delle molteplici proteine acquaporine che stanno fianco a fianco nel complesso epitelio delle vie aeree? Il livello di espressione è strettamente regolato durante lo sviluppo fetale. L'AQP1 e l'AQP3 sono espressi prima della nascita, ma l'AQP4 è espresso solo dopo la nascita. Ecco, quindi, un caso in cui la medicina clinica fornisce un'intuizione scientifica di base. Sono stati preparati topi con geni particolari e questi hanno fornito informazioni importanti. Come medici, tuttavia, siamo interessati alla biologia umana.
Il gene strutturale per l'AQP1 è colocalizzato con un antigene specifico del sangue sul cromosoma umano 7. Come ematologo, sono interessato ad antigeni dei gruppi sanguigni. Anche se ci siano centinaia di diversi antigeni, essi comprendono circa 25 famiglie- e ogni famiglia è un sito genetico. È risultato che l'antigene del sangue Colton (Co) è un polimorfismo semplice sulla superficie della proteina AQP1. Molti di noi hanno gli stessi tipi di antigene del sangue Co, per cui, di solito, non sono clinicamente importanti. Tuttavia, i nostri colleghi dell’International Blood Group Referencing Laboratory di Bristol, Inghilterra, hanno identificato solo alcuni individui privi dell'antigene Co. Così, naturalmente, queste sono le persone di cui ci siamo veramente interessati. Gli individui privi dell'antigene (Co) che siamo stati in grado di studiare erano tutte le donne che, apparentemente, sono diventate "sensibilizzate" durante la gravidanza. Abbiamo determinato che ognuna era omozigote per diverse mutazioni perturbanti nel gene che codifica l'AQP1- ed erano tutte prive dell'AQP1. Il nostro collega Landon King ha studiato due indipendenti individui senza AQP. Membro di lunga data dell’ATS, Landon era uno scienziato ideale per valutare l'AQP1 nel polmone. Gli individui senza AQP1 hanno in circolo degli anticorpi anti-Co e non sono in grado di tollerare una trasfusione di sangue da altri donatori di sangue.

Nonostante l’importanza dell’AQP1, gli individui privi di AQP1 sono in buone condizioni di salute. La medicina clinica, tuttavia, ci fornisce un approccio molto interessante ed efficace per individuare l'importanza delle proteine. Presumibilmente, molti di noi portano mutazioni in diversi geni. Mentre queste mutazioni, solitamente, non causano sintomi, se siamo sottoposti a forte pressione, possono provocare un fenotipo di malattia.
Landon King, in collaborazione con Robert Brown presso la Johns Hopkins, ha utilizzato scansioni con tomografia computerizzata ad alta risoluzione del polmone per valutare la permeabilità all'acqua dei capillari polmonari di questi individui, prima e dopo l'infusione endovenosa di soluzione salina. In figura 9 sono mostrate immagini d’individui normali: un bronchiolo di 2-millimetri e una venula adiacente alla base e le stesse strutture dopo l’infusione di 3 L di soluzione salina fisiologica. È evidente come la venula sia diventata distesa dopo l’infusione del fluido endovenoso, causando una congestione del microcircolo sanguigno. Incrementi simili si ritrovano in tutti gli individui normali e in tutti gli individui privi di AQP1. Dopo l'infusione di soluzione salina, in tutti gli individui normali, la spessa parete dei bronchioli è aumentata a causa del rilascio di acqua dai vasi sanguigni verso e dentro i tessuti molli circostanti. In questo modo, lo spessore della parete bronchiolare è aumentato poiché l'acqua si raccoglie nei tessuti molli perivascolari, formando un edema polmonare allo stadio iniziale. Sorprendentemente, gli individui privi di AQP1 non hanno mostrato alcun rilascio di tale fluido ai tessuti molli circostanti.
I loro bronchioli sono rimasti invariati durante tutto lo studio - evidenziando fortemente che in essi si ha un decremento chiaro e misurabile della permeabilità vascolare all'acqua. A questo punto, possiamo solo fare ipotesi sul significato clinico di questa scoperta. Con il primo respiro di vita, il polmone del bambino assorbe il liquido amniotico attraverso l'epitelio delle vie aeree e questo si trasferisce nei vasi sanguigni circostanti. Per questo, l'AQP1 può essere importante nell'assorbimento perinatale del fluido dal polmone. Se questa è una causa della bassa prevalenza dello stato in cui si è privi di AQP1, allora, presumibilmente, chi sopravvive sta in qualche modo compensando la mancanza di AQP1 nel microcircolo.
Ora, voglio parlare di un’altra acquaporina dei polmoni: l’AQP5 si trova nelle ghiandole secretorie, incluse le ghiandole delle vie aeree che si trovano sotto le mucose, nelle ghiandole lacrimali, salivari e sudoripare. L’AQP5 si manifesta nella membrana apicale delle ghiandole secretorie (Figura 10). La membrana apicale dove risiede l'AQP5 è l'ultima membrana attraverso cui passa l'acqua durante la secrezione di fluidi delle vie aeree, di lacrime, di saliva e sudore. L'evidenza suggerisce che questo è importante nella medicina clinica. Uno studio effettuato in collaborazione con Chris Delporte a Bruxelles e un secondo studio svolto in collaborazione con Kazuo Tsubota a Tokyo ha analizzato le biopsie di un piccolo numero di pazienti affetti da sindrome di Sjo ¨Gren. Questo noto disturbo porta a problemi di occhio secco e bocca secca, così come a disturbi polmonari.

Mentre la sindrome di Sjo¨gren è nota come un problema autoimmunitario, la perdita della funzionalità ghiandolare avviene, spesso, in elementi ghiandolari in cui non si osserva infiltrazione di linfociti. Le biopsie normali hanno un'intensa colorazione anti-AQP5 in corrispondenza della superficie apicale (figura 11, riquadro di sinistra). Al contrario, si può notare la presenza diffusa di proteina AQP5 in tutto l'epitelio ghiandolare (figura 11, riquadro centrale). Questo indica che si è verificato un errore nel trasporto della proteina AQP5, presumibilmente come un difetto secondario. Uno studio indipendente di un maggior numero di biopsie della sindome di Sjo¨Gren è stato iniziato da Roland Jonsson, presso l'Università di Bergen in Norvegia e dai suoi collaboratori. È interessante notare che questi ricercatori hanno trovato una normale distribuzione di AQP5, ma hanno accertato che l'espressione di AQP1 è stata ridotta nelle cellule circostanti mioepiteliali che circondano gli acini ghiandolari. Così, mentre sembra che vi sia eterogeneità, tutti e tre i gruppi hanno mostrato alcune deficienze di acquaporina in quest'importante disturbo clinico, la sindrome di Sjo "Gren, e possiamo solo dunque solo ipotizzare quello che accade nelle ghiandole della sottomucosa del polmone.
Le AQP5 svolgono un altro ruolo importante anche nelle ghiandole sudoripare. In collaborazione con Lene Nejsum e Søren Nielsen, l'analisi delle ghiandole sudoripare è stata eseguita confrontando topi normali con gene AQP5 e topi knockout "elaborati" da Anil Menon e Carissa Krane presso l'Università di Cincinnati. Sebbene entrambi i topi abbiano delle ghiandole sudoripare dall'aspetto normale, i topi privi di AQP5 mostrano delle funzioni ridotte delle ghiandole sudoripare, dopo stimolazione farmacologica.

“GREXIT” is an ugly term for what may soon become an even uglier reality: Greece’s departure from the euro zone. As fury in Athens runs up against frustration with Greek recalcitrance in the rest of the European Union, the EU’s most troubled economy could be heading out of the single currency within weeks. If Greek banks suffer a mass run, as depositors withdraw euros for fear they will be forcibly converted into new drachmas, Greece’s fate could be settled even sooner.
Greece’s ascendant politicians, particularly Alexis Tsipras, leader of the radical left Syriza party, want to repudiate Greece’s rescue deal with its European and IMF creditors. The creditors, particularly Germany, are standing firm, rightly making clear that they will not be blackmailed into repeatedly rewriting bail-outs. If in fresh elections on June 17th the objectors have a majority, as the polls suggest, and if they renege on Greece’s bail-out deal, then the world will cut off the supply of rescue funds. It is hard to see Greece then staying in the euro.
There is already a whiff of inevitability about an outcome once deemed impossible. Central bankers now openly discuss the possibility that Greece may leave. As the impossible lapses into the inevitable, a growing chorus is arguing that it is even desirable. Advocates of an exit say that Greece would gain from a cheaper currency, and that the politics of forging a closer fiscal and financial union between the euro zone’s remaining members would be easier without a country that should never have joined in the first place. But it is wrong to pretend that a Greek exit is an easy or desirable outcome. Before it is too late, Greek politicians need to be honest about what an exit implies. And Europe’s politicians need to act far more boldly to protect the rest of the euro zone in case the worst happens.
Eirexit, Porxit, Spaxit and Ixit
Start with the Greeks. Most of them want to ditch the hated austerity policies that they blame for their plight. Mr Tsipras and his colleagues are fuelling the belief that Greece can somehow avoid austerity and still stay in the euro. In fact Greeks cannot avoid austerity, either within the euro or outside it.
It is true that Greece can survive within the euro only with a gruelling downward adjustment of wages and prices, which demands painful budget cuts and structural reforms. Yet even stronger medicine would be required if Greece left the euro. Cut off from foreign funds, the country would be forced into still tighter fiscal austerity. It would need a disciplined monetary policy and bold structural reforms to retain the gains from its cheaper currency and avoid hyperinflation. Discipline and reform are not familiar concepts in Greek politics.
Moreover a chaotic Greek departure would devastate the country’s political life, because Greece would risk expulsion from the single market and perhaps even the EU itself. A place that shed dictatorship as recently as 1974 would find exclusion from Europe traumatic. For a taste of what might ensue you only need to look at the rising power of extremists such as the neo-Nazi Golden Dawn party.
If Greek voters deserve greater honesty about the Grexit, so do those in the rest of the euro zone. Greece may be a small economy, but a Greek departure from the euro, amid brinkmanship and bluster, would not be a small event. Most obviously, exit—and the subsequent default on its private as well as official debt—would cost European banks, firms and taxpayers a lot of money (see article). And that is without counting the danger of a general contagion in weak euro-zone economies.
There is no formal mechanism for leaving the single currency. As depositors and bondholders across the euro zone factor in the increased risk that their assets could also fall victim to a break-up, other countries would come under pressure. Today’s much-ballyhooed “firewall” is not nearly strong enough.
People strive to avoid disaster. Yet the scope for political miscalculation and financial panic means that the worst might still happen—it may even come soon. Deposits are fleeing Greek banks at an accelerating pace. If financial panic forced a Greek exit before the vote, it would wreck the credibility of pledges that banks across the euro zone are safe. As the Greek economy shrinks within the euro, the economic arguments will become finely balanced—because capital will have fled and the debt burden loom larger. As Greece’s politics is bankrupted, the siren call of populism may grow irresistible.
These dangers require urgent action. First, to prevent a mass run, the European Central Bank must be ready to flood the Greek banks with liquidity—raising the losses to European taxpayers if Greece does eventually leave. And second, to stop a Greek exit being followed by a cascading loss of confidence in other peripheral economies, the euro zone must undergo much faster acceleration towards fiscal and financial integration than most European politicians will admit.
To safeguard banks in Portugal or Spain from runs, European policymakers will have to set up some form of euro-wide deposit insurance. And to reassure investors in the sovereign-debt markets, there will have to be much quicker progress to some form of debt mutualisation among the single currency’s members. The Europeans should have started work on these things during the lull in the crisis earlier this year. Germany resisted that. Now these changes must be done in a rush.
A vote to rock democracy’s cradle
The Greek election is in effect a referendum on whether the country will stay in the euro. It is not completely without hope. A new Greek coalition which vowed to stick to the rescue deal would in fact gain some help from the rest of Europe. At the same time, with the promise of a common banking backstop and some form of Eurobonds, the euro would at last start to look as if it could survive and the dangers of contagion would fall away. And if Greece were an isolated problem, it would be much easier to nurse slowly back to health.
The financial re-engineering of Europe is a prerequisite for the euro to survive. Greece is bringing forward that moment of truth. And yet politicians, particularly in Germany, have still to accept the logic, let alone explain it to voters. The prospect of a Greek exit means they must begin to do so—and fast.

Non è una buona idea per la Grecia lasciare l'euro. Ma è tempo di prepararsi per la sua uscita.
"GREXIT" è un termine poco simpatico per quella che potrebbe presto diventare una realtà ancora più triste: l'uscita della Grecia dalla zona dell'euro. Mentre ad Atene monta la collera contro la frustrazione nel resto dell'Unione Europea per l'ostinata immobilità greca, l'economia più in difficoltà dell'Unione Europea potrebbe dirigersi fuori dalla moneta unica nel giro di poche settimane. Se le banche greche dovessero subire una corsa in massa verso gli sportelli, con i depositanti a prelevare gli euro per paura che siano convertiti forzatamente in nuove dracme, il destino della Grecia potrebbe determinarsi anche prima.

A Guide to Scleral Lens Fitting
Description
This guide is based on an extended literature search on the subject of scleral lens fitting and provides an overview of the latest knowledge and understanding on this exciting vision correction method.
This guide serves as an introduction to scleral shape, scleral topography and scleral lens design as well as a generic guide to fitting scleral lenses to help the practitioner get more comfortable with the concept of scleral lenses. It provides a general overview, supported by the main experienced scleral lens fitters worldwide. Its goal is to give practitioners a framework to oversee and integrate scleral lens fitting into their practices. Being a general overview, it can never cover all of the specific scleral lens designs available and cannot be a fitting guide for all lens types available.
Modern scleral lens fitting still is in its infancy, which makes it a modality with great potential. However, fitting scleral lenses is not very black-and-white, and many differences exist among fitters, cultures, manufacturers and countries. This clinical guide tries to find “common ground” among the mentioned philosophies. For specific lens fitting rules and guidelines, the lens manufacturer and the laboratory’s consultant and specialists have the most knowledge regarding their specific lens design, which practitioners should take advantage of.

In 1967 he started a contact lens laboratory in Australia. He is an expert and a pioneer in scleral lens fitting.
Greg Gemoules, OD – Coppell, TX (USA)
Greg Gemoules received his optometry degree from the Illinois College of Optometry (USA). He moved to Texas and established his practice in Coppell, a growing suburb of Dallas (USA). He has established a large specialty lens practice and has several publications in the peer-reviewed literature. He is a pioneer in the use of optical coherence tomography in the fitting of scleral lenses and has lectured at a number of conferences on the topic.
Tina Graf, BSc – Trier (Germany)
Tina Graf finished her optics degree in 2004, after which she enrolled in the optometry school at Aalen University in Germany, graduating in 2010. During and after her studies she worked at the university hospital in Heidelberg and in different contact lens practices. She conducted a research project at Pacific University College of Optometry on anterior ocular surface shape and presented her data in her thesis and at international meetings.

Rients Visser Sr – Nijmegen (the Netherlands)
Esther-Simone Visser graduated from the School of Optometry in Utrecht (the Netherlands) in 1995. She obtained her Masters Degree at City University in London (UK) in 2004. She joined The Visser Contact Lens Practice, working at several university hospitals in the Netherlands, where she continued to specialize in medical contact lens fitting. She subsequently joined the scleral lens fitting and development team of Rients Visser. She has published and given presentations widely on scleral lenses.
Rients Visser followed the study in Optics, Optometry and Contact lenses in Rotterdam (the Netherlands). He specializes in the medical application of contact lenses and is the founder of Visser Contact Lens Practice, which consists of 19 satellite locations, most of them situated in hospitals. The scleral lens fitting and development team takes care of about 1,700 scleral lens patients. Rients has presented and published widely on scleral lenses and bifocal contact lenses and has developed his own lens designs.

This guide is based on an extended literature search on the subject of scleral lens fitting and provides an overview of the latest knowledge and understanding on this exciting vision correction method. Being an educator, I believe the guide presents an objective, neutral overview that is not biased in any way towards any fitting technique, industry partner or even location— as different approaches exist in different parts of the world. Being slightly at a distance from any specific fitting technology or philosophy felt like an advantage in this process. However, the important feedback from scleral lens experts who work with their specific lens designs and principles on a daily basis was very much desired and appreciated to create a complete overview on scleral lenses. Several visits to large scleral lens practices, interviewing scleral lens experts and discussion forums such as at the sclerallens.org website provided me with great insights.
Trying to merge together the different philosophies and ideas that exist was the most difficult, but also the most rewarding part of creating this guide. Without the input from the international editorial board, I would not have been able to complete this. Not only has the input directly from the contributors and reviewers added tremendously to the content of this guide, their (online) publications and presentations were also invaluable. The International Association of Contact Lens Educators contact lens course modules proved
to be an excellent resource as well —both for understanding the anatomy of the anterior segment as well as for good basic understanding of scleral lenses—and are highly recommended for practitioners. This guide is an update on the latest developments in the dynamic field of this vision
correction method and provides an overview of managing the scleral lens patient.

Questa guida si basa su un'estesa letteratura sull'applicazione delle lenti sclerali e fornisce una panoramica della più aggiornata conoscenza di questo straordinario metodo di correzione visiva.
La guida rappresenta sia un'introduzione alla forma, alla topografia e ai disegni delle lenti sclerali sia una guida generica all'applicazione delle lenti sclerali per aiutare il contattologo a familiarizzare con il concetto di lente sclerale. Grazie al contributo dei più esperti applicatori di lenti sclerali, fornisce un quadro generale sullo stato dell’arte. Il suo obiettivo è di dare agli applicatori dei riferimenti per condurre e integrare l'applicazione delle lenti a contatto sclerali nella loro attività. Offrendo un quadro generale, non potrà mai coprire tutti i tipi di lenti sclerali disponibili e non potrà essere una guida per l'applicazione di tutti i tipi di lenti.
L'applicazione delle lenti sclerali moderne è ancora nella sua fase iniziale; di qui il grande futuro di questo tipo di applicazione. In ogni caso, l'applicazione di lenti sclerali non è assolutamente univoca e vi sono molte differenze fra gli applicatori, fra le culture, fra i produttori e le nazioni. Questa guida clinica cerca di trovare una "base comune" tra le filosofie citate. In merito alle specifiche regole di applicazione delle linee guida e ai particolari disegni, i produttori, i tecnici e gli esperti di laboratorio hanno le maggiori conoscenze e gli applicatori dovrebbero trarne vantaggio.
