заявен Another approach - name/s is / are chosen and given by the parents - very generally speaking. In a document, a name appears as mentioned / referred to, or as given by (the parents)...

Normally you find in this place remarks - if any - like "починал" or "История", so the word has to have some added value.
Could it be that the child is waiting for adoption, or registered at Съвет за закрила на детето? I am just trying to guess...
