The sentence says:
"El paciente indica que radica entre la ciudad de La Paz y Pando, se dedica al transporte y a otros negocios."
There is another part in the form (psychological report) where it says: Domicilio: La Paz y Cobija, so based on that plus the context above, I presume that radicar there means to live.
I've never seen it used that way.
Any comments?

People who can afford multiple homes are often described as "dividing their time between" two places. Example: "Dividing his time between Paris and Berlin, he undertook a series of paintings that he called The Frieze of Life." (Smithsonian Magazine) and another cited below.
Example sentence:
Ex-Justice Gerard, President Wilson\\\\\\\\\\\\\\\'s Ambassador to Germany, ... who is dividing his time between his houses in Switzerland and Paris, ...

I would resolve this by translating the sentence as:
"The patient indicates that he/she lives in both (the cities of) La Paz and Pando..."
You can't really translate "entre" here as between, as that would imply in English that the patient does not live in either city, but somewhere between the two locations.

Note: I really would discourage the use of "between" for "entre" in this case, as it could be interpreted the mean that the individual lives somewhere between both cities, but in neither of them. For example, if I were to say "I live between Philadelphia and New York," that could be taken to mean that I actually live in New Jersey (which is, after all, between those two cities). I think that using "in both" instead of "between" resolves this potential ambiguity.
