emotional (anxiety/stress from) separation

I think it refers to the emotional separation from drugs, that is to say, while you can deal easily with the pysical aspects of kicking thehabit, the emotionally loss associated is harder to deal with.

Without more context, it's hard to tell.

letting go emotionally

I agree the question is ambiguous, as it could either refer to drugs or a relationship. My suggestion is vague enough to apply to either. That said, I would assume this is a poetic text which refers to a relationship, otherwise they would not refer to drug detox in such light terms.
