"Minnesblad" is the title of a document that is otherwise a certificate of baptism. The meaning is clear to me - however, any ideas for an adequate translation in this context?

If this is a one page document, you could say "Commemorative leaflet".

'Commemorative pamphlet' or 'commemorative booklet' could be used for multi-page documents.

disagree De Novi : It is not a certificate in itself, it's something that comes along with the certificate

That's what I would expect too, but asker says "a document that is otherwise a certificate of baptism".

This is a hard one, but since it used when baptism, weddings, funeral etc, I guess this is close enough =)
