Actually, I met a rational hospital administrator in a small town in Peru. She asked for my second last name, and when I told her I had none, she asked "Mother's last name?". I told her, and presto, she had her form accurately filled out. 'Smart bureaucrat' is not always a contradiction in terms.

agree Andy Watkinson : Better than what happened to a friend of mine with one surname attempting to convince a stubborn funcionario to leave a blank space on the form. Total refusal, funcionario’s solution was to write in the corresponding box: "de madre desconocida"
