INTRODUCTION
You have already agreed to take part in the main study named above by signing the Assent Form. You are now being asked to take part in a separate, optional part of the study (a sub-study) that involves providing a sample of blood so that the sponsor of the sub-study can analyze your genes to learn how differences in your DNA may affect how you and others respond to the study drug.
This form only gives additional information for you to decide if you want to take part in this optional sub-study. You may stay in the main study even if you decide not to be a part of the sub-study and do not want to give a sample and sign this form.
