المعنى الحرفي هو "الصداع" لكن يقصد به المتاعب هنا، أو ما يسمى أحياناً "وجع الرأس" ـ

Why should we consider the translation of "headache" apart from its semantic unit?

"Who gets headache from" could be simply rendered as a verb that functionally communicate the message to the target receiver.

Hence, I suggest

الذين يتضجرون/ ينزعجون/ يعانون
and there are many other verbs that serve the same purpose
