It appears on the same (tax) form from Brazil I've asked about earlier.
The copy is not good, but it seems to say:
"declaração ou retificadora?" (with the person's response being "não").

On re-reading your context, I realize that the person answered "no." Is there any chance that there is something besides "ou" in your text? "Declaração retificadora" would simply mean amended return (or declaration).
