Ange dag/månad/år taget

Swedish to English Medical Medical (general)
In an instruction booklet for a patient dosage diary, it says, "Ange dag/månad/år taget".

Is this "enter each day/month/year"? or is this short for enter one "day/month/year at a time."

"Taget" must refer to when the medicine has been taken.

agree Cynthia Coan : This makes sense; enter entire date each time one takes a medicine. The dd/mm/yy date format is commonly used in Europe.
