Era un vecchio medico molto noto e stimato in città che, grazie alla sua buona sorte ma anche a un'oculata gestione delle sue amicizie e frequentazioni, era riuscito miracolosamente a passare indenne attraverso le maglie dell'Inquiszione anche se in odore di stregoneria, dato che preparava da sé—e quel che è peggio in grande segretezza!—la maggior parte dei portentosi farmaci che sominstrava ai suoi pazienti.

Another option would be to keep the same figurative image of the original as in "odo(u)r of witchcraft", which clearly refers to "odo(u)r of sanctity" of Catholic origin.

The wily old doctor was miraculously successful at (passing through the mesh of) breaking down the shackles of the inquisition even though he did it with a hint of witchcraft/ magic, given that the potions he prepared by himself formed the largest part of the landmark medicines he supplied to his patients,( and what is worse, something which he did in great secrecy)
